This article is designed to help Evolv workers to properly complete a Treatment Plan for a client.
Important Notes:
- Please read through all important information before continuing with this article
- Please make sure to follow all steps as written below. If there is any issues with this article please report them to the IT Department immediately for corrections and additions.
IMPORTANT: See treatment plan instructions for more details. Treatment plan instructions**** will only show items I have selected and will not show unchecked items due to printing feature (with the exception of goal 2 and goal 3- these sections are blank due to instructions not including a goal 2 or 3)
- No longer under IAP Section in Evolv
- No longer need to complete Strengths & Needs sections in Evolv (this is embedded in the new treatment plan form)
- Treatment Plan Breadcrumbs: Client- Case Management- Service Management- Service Entry- Add Event- Treatment Plan
Instructions:
- To be reviewed by section: enter in your supervisor
- Demographics have a feature to automatically populate into your treatment plan form
- If demographics are not listed in the treatment plan, it means there are no demographics entered into Evolv- you can enter in at this time using the treatment plan form
- Diagnosis: diagnoses from other programs, including list of diagnoses no longer pre-populate in our treatment plans in order to be client-centered and to ensure we are only utilizing current, active, diagnosis that we are treating at this time
- When selecting diagnosis- a menu of the client’s “active” diagnosis will show- select the appropriate diagnosis you have diagnosed/are treating from this list
- You can update/adjust client’s diagnosis as appropriate under active diagnosis
- Current medications: will automatically populate from data entry
- Type of Treatment Plan: select: Initial Treatment Plan or Treatment Plan Update: see below for details
- You can select Initial Treatment Plan if this is the client’s first treatment plan and if this is the first time you are using this treatment plan form
- Save Draft: this feature at the top right: to be used if you need to abruptly pause completing treatment plan with your client and you are not able to complete all required fields- Save Draft feature allows you to save your work to date
- Save: you need to click Save at the top left in order to move forward with submitting/signing your treatment plan (if you previously hit Save Draft- you will need to go back into the treatment plan form and hit Save before having the ability to Submit for approval)
- Clicking “Save” is the last step prior to submitting treatment plan for approval
