This article will assist Evolv workers how to use the Legal Custody form in Evolv to document legal custody events and track the guardianship and custodial rights for a client or referral.
Important Notes:
- Please read through all important information before continuing with this article
- Please make sure to follow all steps as written below. If there are any issues with this article, please report them to the IT Department immediately for corrections and additions.
- It is important to review any/all existing active guardians in a client's chart before adding additional guardians.
- If client has more than one guardian, list each guardian separately by using "Add New" for each guardian name.
- Use "End Date" to end a listed guardian before adding a new/different/updated guardian.
- Guardian information may be required within specific Evolv forms.

- Client > Client Information > Relationships > Guardianship/Custody Information
- Referral > Agency Referral > Relationships > Guardianship/Custody Information
- Program Referrals > Program Referrals > Relationships > Guardianship/Custody Information
Instructions:
- Go to one of the above listed modules, then search for and select a client or referral.
- Select Add New > Add Event, then choose Guardianship/Custody Information:
- Entered With - (non-modifiable) will populate where the guardian information was entered. i.e., agency placement, specific form, referral, etc.
- Event - (non-modifiable) will populate "Guardianship/Custody Information" as event name
- Guardianship Type - Select a type of guardian, such as relative, parent, etc.
- Temporary Guardianship - check if guardianship is temporary
- Permanent Guardianship - check if guardianship is permanent
- Guardianship Name - enter name of legal guardian
- Agency Awarded Legal Guardianship - search and select agency (when applicable). i.e., DCF, Court, etc.
- If search does not result with exact agency name, submit a helpdesk ticket with request to add agency.
- Ticket will need to include:
- Agency legal/full name
- Agency address
- Agency phone number
- Agency fax number (f applicable)
- Agency website link
- Ticket will need to include:
- If search does not result with exact agency name, submit a helpdesk ticket with request to add agency.
- Guardian Relationship to Client - search and select relationship type of guardian to client. i.e., mother, father, aunt, etc.
- select "Self" for adults who are their own guardians.
- Guardian Phone Number - enter guardian's phone number.
- Guardian Mobile Number - enter guardian's mobile number.
- Guardian Alternate Number - enter guardian's alternate phone number.
- Effective Date - select from calendar or enter date documenting guardian information (usually today's date).
- End Date - select from calendar or enter date listed guardian ended (usually today's date).
- Scanned Document - use to upload guardianship related documents.
- Proof of Guardianship - use to upload proof of guardianship document
- Additional Information - use Remarks to document additional guardianship information.
- Select Save.
