This article explains how to complete the Collateral Contact Note in Evolv for the CSA, IHT and TM programs
Important Notes:

- This article reviews how to complete a Collateral Contact Note for the following programs:
- Community Service Agency - Day Rate
- Community Service Agency - Family Support & Training
- In Home Therapy
- Therapeutic Mentoring
- Service Explanation for Collateral Contact for CSA/IHT/TM programs
- Defined as a face-to-face or telephone, telehealth communication between the worker and an individual or agency regarding treatment planning coordination, or medical necessity.
- Use with all providers except within the same CFS program.
Instructions:
- Navigate to Client > Case Management > Service Management > Service Entry
- Select Add Event in the correct program enrollment
- Select Collateral Contact event
- Complete the following form fields - NOTE: * indicates a required field
- Service Information
- Program Enrollment - leave blank
- Client - auto populated with client name
- Type* - auto populated with event name
- Actual Date/Time* - auto populated with today's date/time; this can be modified if Collateral Contact occurred during a different date/time.
- Duration* - enter duration of Collateral Contact
- Program Providing Service* - auto populated with program name selected within Service Entry
- Site Providing Service* - auto populated with site client is enrolled
- Type off Scheduled Contact
- check box which represents how Collateral Contact was performed.
- Purpose
- Check box(s) indicating the purpose of the Collateral Contact
- Use "Other Purpose" if available choices do not include the purpose needed; add the purpose reason when choosing "Other Purpose"
- Check box(s) indicating the purpose of the Collateral Contact
- Progress Note
- Progress Note* - write detailed information of Collateral Contact encounter
- Scanned Documents
- Scanned Document Submitted - check box is a document, related to this Collateral Contact, was submitted for scanning
- Collateral Contact Document
- Click to upload a document related to this Collateral Contact
- Service Information
- Choose "Save Draft" if Collateral Contact note is incomplete but need to exit
- Choose "Save" when entire note is complete - NOTE - all required fields must be completed before SAVE is selected
- E-Sign once note is Saved
